You Will
Wellness Program Member Support
• Guide members through Empowered Health wellness programs from onboarding through completion
• Help participants understand program structure, expectations, and milestones

• Monitor participant progress and help members stay engaged throughout the program

• Provide evidence-informed nutrition education aligned with Empowered Health programs

• Answer nutrition-related questions within the scope of program protocols
• Reinforce physician recommendations related to lifestyle medicine and wellness
Program Coordination & Execution
• Coordinate day-to-day logistics of wellness programs
• Support onboarding of new program participants
• Ensure program materials and resources remain organized and consistent
• Maintain clear documentation of member progress and program participation
• Work with leadership to improve workflows and program delivery
Program Outcomes & Progress Monitoring
• Track participant engagement and completion rates
• Monitor program progress and identify participants who may be falling behind
• Own internal reporting of program metrics and outcomes
Collaboration with Clinical Team
• Work closely with physicians, care managers, and practice leadership
• Participate in weekly program meetings reviewing participant progress and member experience
• Participate in full team meetings to maintain alignment across the practice
• Escalate clinical concerns to physicians when appropriate
Program Growth & Education
• Learn how Empowered Health wellness programs are presented to prospective participants
• Observe and gradually participate in program education conversations with members

Who You Are
You’re likely:
• Registered Dietitian (RD/RDN), nutritionist, or strong training in nutrition and lifestyle medicine

Your Role & Responsibilities
This is a full- or part-time in-clinic position (with hybrid options over time). You’ll join a small, elite team in a purpose-built clinic.
You Will:
- Manage a thoughtful panel of engaged patients (average 4-8/ half day)
- Conduct extended visits (30–90 min) with a focus on lifestyle, labs, and root cause
- Collaborate with a team of coaches, nurses, and like-minded practitioners
- Use genetic, biomarker, and diagnostic data to craft precise care plans
- Help patients reverse chronic disease, prevent decline, and optimize vitality

Care Manager (Part-Time)

We have stepped outside of the conventional medical model to return to the core values of the doctor-patient relationship, allowing for time, trust, transparency and technology to be used optimally for patient care.
You:
We are looking for an experienced, self-motivated Care Manager (medical receptionist) to join our growing medical practice. An ideal addition to our team is an individual who is detail-oriented, organized and enjoys working in a fast-paced environment. This individual would also be proficient in problem solving and providing exceptional customer service to ensure smooth day-to-day execution of our clinical care.
You will be required to work physically in the clinic. The Care Manager role is integral to patient care and your physical presence and regular attendance during your scheduled shifts are required.
A part-time employee will work 20 hours/week.
Our ideal candidate will have at least 2 years of experience working in a healthcare setting with scheduling experience.
You Will:
- Greet members, along with checking them in and out from office visits
- Manage the appointment calendar, including scheduling and rescheduling member appointments
- Manage the member onboarding and offboarding experience
- Understand, explain and dispense all tests and supplements to members per the doctor’s treatment plan, including a thorough explanation of testing instructions and billing procedures
- Assist members with insurance and prescription coverage needs
- Manage inbound member inquiries from our online messaging platform
- Provide seamless care coordination over messaging to ensure a smooth patient experience
- Proactively ensure clinicians are prepared for their member visits each day
- Coordinate collection and proper distribution of outside records or test results
- Follow up on orders and prescriptions as requested by medical staff
- Prepare and clean exam rooms and medical equipment; assist medical providers with patient care, as appropriate
- Tidy, organize and monitor the physical appearance of the clinic daily; with larger cleaning tasks done on a schedule and as needed
- Work closely with all members of the Empowered Health team to optimize our product to deliver an
amazing member experience and ensure provider happiness - Understand and confidently communicate Empowered Health’s products and offerings
- Uphold a high standard for member experiences in all interactions
- Contribute to process improvement; take initiative to seek, solve, and innovate creative solutions
You Have:
- 2+ years of work experience, ideally in healthcare operations, medical assisting or care coordination
- Availability to work on location during scheduled shifts
- Demonstrable organizational skills and attention to detail
- Experience using an electronic medical record system
- Enthusiasm for wellness and functional medicine and a positive, go-getter attitude
